Summary

The Advanced Concepts in Functional Capacity Evaluation course is designed for experienced Functional Capacity Evaluation practitioners who want to critically examine and strengthen their current FCE methodology, clinical reasoning, and professional opinions. The course is intentionally collaborative and thought-provoking, challenging participants to analyze established habits and perspectives while considering available research literature, clinical experience, and the perspectives of other experienced evaluators.

Building on prior FCE training and experience, the course explores advanced issues related to effort analysis, validity, work tolerance, repetitive movement testing, pain and disability reports, lifting biomechanics and safety endpoints, productivity, repeat functional testing, work simulation, and challenging client presentations. Emphasis is placed on examining not only how FCE procedures are performed, but whether the methods selected and conclusions drawn are clinically meaningful, appropriately supported, and defensible.

Participants examine the pursuit of validity within FCE and the importance of matching evaluation activities to the actual demands of the target work. The course challenges practitioners to consider whether standardized testing adequately represents job requirements and when evaluation methods should be modified or supplemented to improve content and ecological validity.

Special attention is given to the interpretation of pain and disability reports and the evaluator's responsibility to explain how reported limitations relate to objectively measured and observed function. Participants also examine the use and misuse of repetitive movement testing, functional change over time, and the interpretation of declining, stable, or improving performance during an FCE.

Throughout the course, participants apply the Matheson Practice Hierarchy of Safety, Reliability, Validity, Practicality, and Utility to advanced evaluation decisions. The emphasis remains on the Matheson-Trained Thinking Evaluator, using research, individualized clinical reasoning, and the referral question to determine what should be tested, how findings should be interpreted, and how the evaluation can contribute meaningfully to case resolution.

The course also addresses work simulation, critique of FCE methodology and reporting, and the communication and defense of professional opinions. Participants develop strategies for identifying weaknesses in case work and their own practice and for responding to common challenges encountered during expert testimony and cross-examination.

Target Audience:

Physical Therapists, Occupational Therapists, Athletic Trainers, and other licensed rehabilitation professionals involved in work rehabilitation, functional restoration, and return-to-work decision-making.

Completion of formal Functional Capacity Evaluation training and practical experience conducting or interpreting FCEs.

  • Incorporate work simulation into FCE methodology to make functional testing more meaningful and convincing.
  • Develop a critique pathway to identify and address deficiencies in case work and improve one's own FCE practice.

Chapter 1: Advanced FCE Philosophy, Effort Analysis, and Reliability (90–100 minutes)
  • Application of the Matheson Practice Hierarchy of Safety, Reliability, Validity, Practicality, and Utility to advanced FCE decision-making
  • Research related to effort analysis and the interpretation of statistical thresholds during functional testing
  • Distinguishing measured performance from assumptions regarding effort, motivation, or intent
  • Integrating multiple sources of evidence when evaluating consistency and reliability of performance
  • Developing conclusions that clearly communicate what the available evidence does and does not support
  • Learning emphasis: Challenging established FCE practices by examining whether conclusions regarding effort and reliability are supported by the available evidence and appropriate clinical reasoning.
Chapter 2: The Pursuit of Validity and Job-Specific FCE (90–100 minutes)
  • Content and ecological validity as foundations for meaningful functional assessment
  • Matching FCE activities with the physical and cognitive demands of the target work
  • Limitations of highly standardized protocols when test demands do not adequately represent actual work
  • Modifying test selection, performance parameters, frequency, height, duration, and other variables based on job demands
  • Balancing validity with the practical constraints and utility of the evaluation
  • Applied activity: Critique an FCE testing plan and determine where standardized procedures should be retained, modified, or supplemented to better represent the target work.
Chapter 3: Work Tolerance and Repetitive Movement Testing (75–85 minutes)
  • Challenges associated with translating point-in-time performance into opinions regarding sustained work tolerance
  • Appropriate interpretation of occasional, frequent, and constant functional demands
  • Origins, intended purposes, and limitations of repetitive movement testing within FCE
  • Functional Baseline and Repeat Testing as a method for examining performance over time
  • Interpreting functional improvement, stability, and decline in relation to pain, fatigue, behavioral factors, and normal performance variability
  • Applied activity: Review repeated functional performance data and determine what conclusions regarding work tolerance and performance over time can reasonably be supported.
Chapter 4: Pain, Disability Reports, and Functional Performance (75–85 minutes)
  • Pain as a subjective experience and the importance of evaluating its reported functional consequences
  • Limitations of pain-intensity measures when determining functional impact
  • Use of function-based pain measures within Functional Capacity Evaluation
  • Comparing reported abilities and limitations with measured and observed functional performance
  • Developing clear and convincing explanations regarding the reliability of pain and disability reports
  • Applied activity: Integrate symptom reports, functional observations, and measured performance to formulate a defensible opinion regarding the reliability and functional impact of reported pain.
Chapter 5: Lifting, Biomechanics, and Safety Endpoints (90–100 minutes)
  • Review of available literature related to lifting biomechanics and safe functional testing
  • Evaluator observation and clinical judgment when determining safe maximum lifting performance
  • Distinguishing physical effort from biomechanical or clinical indicators requiring modification or termination
  • Considering individual movement strategies and the relationship between lifting technique, load, and functional capacity
  • Examining whether evaluator intervention may prematurely limit the measurement of lifting capacity
  • Applied activity: Analyze lifting scenarios and determine when continued testing, modification, or termination is supported by the observed safety and performance findings.
Chapter 6: Work Simulation and Functional Relevance (90–100 minutes)
  • Role of work simulation in improving the functional and occupational relevance of an FCE
  • Moving beyond standardized tests when they do not adequately reproduce critical job demands
  • Designing simulations that reflect relevant task demands, environments, postures, handling requirements, and performance expectations
  • Balancing realism, safety, standardization, practicality, and available resources
  • Using work simulation findings to strengthen conclusions regarding functional capacity and work ability
  • Applied activity: Develop a work simulation that addresses a critical job demand not adequately represented by standardized FCE testing.
Chapter 7: Productivity and Repeat Functional Testing (75–85 minutes)
  • Productivity as an important dimension of functional work performance
  • Using repeat functional testing to examine performance across the evaluation
  • Comparing baseline and later performance to identify improvement, stability, or decline
  • Distinguishing meaningful functional change from normal variation in human performance
  • Integrating productivity, endurance, symptoms, and performance consistency when developing work-capacity opinions
  • Applied activity: Interpret repeat functional findings and determine whether the observed pattern supports conclusions regarding productivity, endurance, and sustained work performance.
Chapter 8: The Difficult Client and Behavioral Overlay (75–85 minutes)
  • Recognizing challenging behaviors and presentations that may complicate FCE administration and interpretation
  • Separating observable behavior from assumptions regarding motivation, intent, or character
  • Maintaining clinical balance and professional objectivity when findings are inconsistent or difficult to interpret
  • Managing evaluator-client interactions while maintaining safety, respect, and the integrity of the evaluation
  • Documenting behavioral observations in language that is clinically relevant, objective, and defensible
  • Applied activity: Review a challenging FCE presentation and develop an objective interpretation and documentation strategy without assigning unsupported motivation or intent.
Chapter 9: Critique Path and Improving FCE Practice (60–70 minutes)
  • Systematic critique of FCE methodology, analysis, conclusions, and reporting
  • Identifying gaps between the referral question, job demands, testing methods, and conclusions
  • Evaluating whether opinions are adequately supported by objective findings and clinical reasoning
  • Recognizing common weaknesses that reduce the validity, defensibility, or utility of an FCE
  • Applying the critique process to improve one's own evaluation methodology and reporting
  • Applied activity: Critique an FCE case or report and identify specific methodological, analytical, and documentation changes that would strengthen the evaluation.
Chapter 10: Expert Testimony and Defending the FCE (60–70 minutes)
  • Translating FCE methodology and findings into professional opinions that can be clearly explained and defended
  • Anticipating common challenges to test selection, methodology, interpretation, and conclusions
  • Responding to questions regarding reliability, validity, safety, work relevance, and limitations
  • Recognizing the importance of acknowledging uncertainty and the boundaries of available evidence
  • Maintaining professional objectivity when opinions are challenged during testimony or cross-examination
  • Applied activity: Practice responding to common challenges to FCE methodology and conclusions through simulated questioning and discussion.
Applied Discussion
  • Applied discussion is integrated throughout the two-day program to challenge established FCE habits and reinforce the development of the Matheson-Trained Thinking Evaluator. Discussion focuses on how available research, clinical findings, job demands, client presentation, and evaluation purpose should influence testing decisions and interpretation. Participants are challenged to determine what information is necessary to answer the referral question, recognize the limitations of available evidence, and formulate conclusions that are clinically meaningful and defensible.
Course Assessment
  • Assessment is designed to evaluate advanced clinical reasoning and applied decision-making rather than recall of a fixed FCE protocol. Participants analyze cases, critique FCE methodology and reporting, interpret effort and repeat-performance findings, develop work simulations, defend clinical conclusions, and practice responding to challenges regarding their evaluation methodology and professional opinions.
Required Materials
  • The Advanced Concepts in Functional Capacity Evaluation Course Manual, course case and critique materials, course presentations and supporting research literature, and writing materials or an electronic device for documentation and case exercises are required. Participants should also have access to foundational FCE testing materials and resources as directed by the instructor. Live-stream participants will need reliable internet access and a device capable of accessing the course platform and participating in interactive activities.
Clinical Reasoning Reinforced Throughout the Course
  • Clinical reasoning is reinforced by challenging participants to question whether established FCE practices actually produce safe, reliable, valid, practical, and useful information. Participants examine the evidence supporting their testing decisions, distinguish measured findings from inference, adapt evaluation methods to the referral question and target work, recognize uncertainty and limitations, and develop conclusions that can withstand clinical, administrative, and legal scrutiny. The emphasis is on becoming a more thoughtful and defensible evaluator rather than simply adding additional tests to an existing FCE protocol.
