Summary
The Cognitive Functional Capacity Evaluation (Cog FCE) course prepares rehabilitation professionals to systematically evaluate how cognitive, physical, behavioral, and psychosocial factors influence an individual's capacity to perform work and other meaningful occupational activities.
Building on the principles of Functional Capacity Evaluation, the course expands the evaluation process beyond traditional physical demands to examine functional cognition in the context in which it matters most: real-world performance. Participants learn to evaluate the relationship between an individual's demonstrated abilities and limitations and the cognitive, behavioral, physical, and environmental demands of work.
The course introduces a structured Cognitive FCE methodology integrating medical and occupational information, job demands analysis, standardized and non-standardized assessment, structured observation, cognitive and physical effort evaluation, work simulation, and clinical reasoning. Particular attention is given to executive functioning, attention, mental stamina, fatigue, insight and awareness, task persistence, compensatory strategies, and performance over time.
Participants learn to identify the critical cognitive and psycho-emotional demands of a job, select assessment methods that reflect those demands, and design work simulations that increase the ecological and occupational relevance of the evaluation. The course also addresses interpretation of subjective reports in relation to observed performance and emphasizes a multi-method approach to evaluating physical and cognitive effort, rather than relying on a single test or observation.
Throughout the course, participants apply the Matheson philosophy of the Thinking Evaluator, using the Practice Hierarchy of Safety, Reliability, Validity, Practicality, and Utility to guide assessment decisions. This approach emphasizes respect for the person being evaluated, best practice, flexible clinical reasoning, and case resolution rather than reliance on a rigid, recipe-based testing protocol.
Through case studies and applied clinical reasoning, participants learn to integrate subjective and objective findings into defensible conclusions regarding functional cognitive capacity, work tolerance, return-to-work potential, accommodations, rehabilitation needs, and recommendations for further assessment or intervention.
The goal is not simply to identify cognitive impairment, but to determine how cognitive abilities and limitations affect functional performance within the actual demands of work and everyday life.

Prerequisites:
Completion of formal Functional Capacity Evaluation training. This does not need to be from Matheson, but may cause less confusion on vernacular and thought processes compared to other Functional Capacity Evaluation methods.

- Applied discussion is integrated throughout the course using complex Cognitive FCE cases. Participants identify essential cognitive and psycho-emotional job demands, determine appropriate assessment strategies, select patient-reported and standardized measures, design work simulations, evaluate effort and reliability, and integrate subjective and objective findings to formulate opinions regarding functional capacity and return to work. The course emphasizes reasoning through the assessment process rather than applying a predetermined battery of cognitive tests.
- Assessment includes knowledge checks, case-based discussion, test-selection exercises, cognitive job-demand analysis, work-simulation development, and integrated case interpretation. Participants are expected to demonstrate the ability to select appropriate assessment methods, interpret findings within the occupational context, evaluate functional cognitive performance, and support conclusions regarding work capacity and return-to-work readiness.
- The Cognitive Functional Capacity Evaluation Course Manual/Handout, course worksheets and case materials, course presentations and supporting instructional resources, and writing materials or an electronic device for documentation and case exercises are required. Participants should have access to the assessment tools and occupational-demand resources identified for applied activities, as directed by the instructor. On-demand and live-stream participants will also need reliable internet access and a device capable of accessing the course platform.
- Clinical reasoning is reinforced by requiring participants to move continuously between the person, symptoms, cognitive and psycho-emotional function, occupational demands, observed performance, and assessment findings. Rather than relying on a single cognitive score, participants learn to determine where evidence converges or diverges, evaluate performance under increasing and sustained demands, and translate the complete pattern of findings into a defensible opinion regarding functional capacity and return to work. This reflects the comprehensive methodology described in the manual, which culminates in analysis, clinical reasoning, formulation of opinion, documentation, and recommendations.
